Our Facilities
Congregate Living
Our supported group living and acquired brain injury programs provide long-term housing and support for individuals with complex developmental and medical needs
Supported Independent Living
Our apartment and community programs provide person-directed support for individuals living in the community
Outreach
The Outreach program is designed to help self-directed individuals who require some assistance to be able to remain in their own homes. Community members accessing the Day Away program through HCSS have opportunities for bathing through an offshoot of Outreach as well.
