Our Supported Independent Living program provides 24/7 support to individuals residing in community homes. Currently with three homes in the Owen Sound area, our staff assist with tasks of daily living, varying in degrees of support, based on our supported individual's needs. To ensure person centred care, we develop individualized support plans in collaboration with our Supported individuals, and those most important to them, to ensure we create a plan designed to help them reach their goals, participate in their community, continue to gain skills and enrich their lives.
